We’re delighted that you would like to register with our practice!  Arnie and Gayle Agnew founded Agnew Equine in 2001, and we are still an independent, family-owned business which has built a reputation for being trustworthy, professional and compassionate.  We recognise that choosing us to care for your horse or pony is an important decision and we’ll make every effort to give you a service that not only delivers on your expectations, but exceeds them. 

The best way for us to collect information about you and your horse is for you to fill out the form below -  but, feel free to call the practice on 01782 510502 to let us know you’d like to register and to ask any questions. Regardless, we’ll  be in touch and will also send you a Welcome Pack full of useful information after you register.

Can We Stay In Touch?

Changes to legislation from 25th May 2018 mean that you will no longer hear from us unless you confirm you’re happy for us to contact you. If you are happy to hear from us - for example to receive vaccination reminders, info on local disease outbreaks, or events such as client meetings or new services - just let us know which is your preferred method:

HORSE DETAILS

Please provide us with details of any current veterinary treatment your horse is receiving, any drug allergies or any other information you would like us to be aware of.
Would you like us to request a copy of your horse’s medical records from your previous vets? If yes, please provide us with their details.

Providing us with the following information will allow us to generate a reminder for your horse’s next vaccination:

If your horse is vaccinated, what date was the last vaccination given on?

If yes, please proceed to our ‘Book an Appointment’ page, but only AFTER you have completed and submitted this registration page. If you are looking to book a vaccination, don’t forget to look at our Saver Day options where you can combine this with a very affordable dental examination.

To do our part for the environment, are you happy to receive any invoices from us via email instead of by post?

If you need to add more than one horse to your registration, please submit a complete registration form for the first horse. After submitting that, please return to fill out a new form for each additional horse, but you only need to enter your email address and the horse’s details when you do so.
